Care Manager(Full-time)

Care Manager

The responsibilities ofthe Care Manager position include, but are not limited to direct casemanagement, participation in care coordination via the treatment team andsupervision of Shared Living and Adult Family Care (AFC) services. A clear andcomprehensive understanding of DDS/ABI, AFC and other community/ home caredelivery services preferred. The ability to adapt to changes, modify andredirect processes is required.  

This position combines ahybrid working model with ability to attend in person visits to homes and tomeetings.

Requirements

·      Bachelor’s degree from an accredited college required

·      Two (2) years recent experience working with the elderpopulation or those having a disability

·      A broad working knowledge of DDS or Adult Family Care community-based services is preferred

·      Computer skills, organizational planning, travel, as well asexcellent communication skills and team building skills are necessary
