We believe in the vision of life sharing and its transformative power of caring for a person in a home-setting, Nonotuck provides personalized supports to people by matching them with caring providers who open their hearts, their minds and their homes.
Just shy of 1,200 people receive Caregiving with Love in either Shared Living or Adult Family Care. Position Overview:
Nonotuck Resource Associates is seeking compassionate and dedicated individuals to join our team as Direct Care Professionals. As a Direct Care Professional, you will play a vital role in providing high-quality supports to individuals, fostering community engagement, and promoting their overall well-being. Direct Care Professionals at Nonotuck Resource Associates provide leadership in implementing the organization's values and key principles to deliver high-quality support to individuals. This role involves understanding the unique strengths and needs of each person supported, assisting in goal development, and supporting the individual's community-based daytime life.
Facilitate engagement in person specific community activities which may include going out to eat, attending dances, bowling, playing bingo, attending concerts, cooking and learning new skills, yoga, Zumba and going to yard sales.
Key Responsibilities:
- Individual Support
- Develop an understanding of each person supported, their unique strengths, needs, goals, and desires.
- Assist individuals in articulating and achieving their needs, goals, and desires.
- Community Integration
- Work as a member of a team to support individuals in developing a full community-based daytime life, including skills and experiences associated with it.
- Promote and support community engagement and participation.
- Positive Image and Relationships
- Support and exert personal initiative to ensure that each person presents a positive image of themselves and the program.
- Assist individuals in developing long-lasting and meaningful relationships with family, friends, community members, and co-workers.
- Community Activities
- Assist individuals in participating in community activities, including group membership, volunteer commitments, educational opportunities, leisure, and social pursuits.
- Individual Service Plan (ISP)
- Complete Individual Service Plan goals and activities.
- Monitor and provide assistance (direct care) with Activities of Daily Living (ADL) as needed.
- Team Collaboration
- Work closely with Program Director, Assistant Program Directors, and Care Managers.
- Regularly monitor Nonotuck email inbox and receive information via email.
- Flexibility and Ongoing Education
- Perform any and all duties deemed essential to support the agency, office, team, or people we support.
- Ability to drive a motor vehicle to the home(s) of people served.
- Availability to work weekends, with the schedule determined by the person served.
- Participate in ongoing educational information and trainings on health, aging, and specific member needs.
Qualifications:
- High school diploma or equivalent (additional education and training in human services is a plus).
- Previous experience in direct care or related field is preferred.
- Excellent interpersonal and communication skills.
- Empathy, patience, and a commitment to promoting independence and community inclusion.
- Community experience that expands specifically to the Department of Developmental Services, Adult Foster Care Services, HCBS waivers (supporting those with a Brain Injury or those who have chronic conditions affecting their availability to live safely in the community) preferred
- Background check required.
- Valid driver's license with an excellent RMV record (subject to RMV background check).
