Just shy of 1,200 people receive Caregiving with Love in either Shared Living or Adult Family Care. AFC Care Manager (Full-time Exempt, Monday-Friday 40 hours/week)

Pay Range: $55,000 Annually

Key Responsibilities:

  • Assessment and Evaluation
    • Conduct initial and ongoing assessments of AFC qualified settings, including interviews, home studies, home inspections, and medical clearance of care providers.
    • Obtain social history and conduct psychosocial assessments and evaluations.
  • Caregiver Management
    • Select, train, evaluate, and supervise AFC caregivers in collaboration with registered nurses.
  • AFC Plan of Care
    • Participate in the development, implementation, and ongoing review of the AFC plan of care.
  • On-Site Visits
    • Conduct on-site visits with each member at the qualified setting bimonthly for Level I and monthly for Level II, alternating with the registered nurse.
  • Documentation and Progress Notes
    • Complete a care management progress note corresponding with each on-site visit or encounter and upon significant change.
    • Periodically review AFC caregiver logs.
  • Health and Community Services
    • Assist with obtaining information and accessing other health-care and community services for individuals under care.
  • Safety Procedures and Discharge Planning
    • Review and document fire and safety procedures for qualified settings.
    • Participate in discharge planning and implementation.
  • Annual Review
    • Review at least annually the suitability of the qualified setting.
  • Additional Duties
    • Complete intake forms in conjunction with the registered nurse.
    • Prior to placement, complete all required Nonotuck paperwork for both care provider and person served in conjunction with the RN.

Qualifications:

  • Bachelor's Degree, Social Work License, and two years of experience working with elderly or disabled adults.
  • Knowledge of AFC regulations and guidelines.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively with a multidisciplinary team.
  • Attention to detail and organizational skills.
  • Valid driver's license with an excellent RMV record (subject to RMV background check).
  • Background check required.
