Nonotuck Resources Associates (www.nonotuck.com) is a community-based agency providing residential services to people with disabilities since 1972. We believe in the vision of life sharing and its transformative power of caring for a person in a home-setting, Nonotuck provides personalized supports to people by matching them with caring providers who open their hearts, their minds, and their homes.

Just shy of 1,200 people receive Caregiving with Love in either Shared Living or Adult Family Care. Each person is surrounded by loving, dedicated, experienced and compassionate team members who provide values-based services driven by personal preference, community inclusion, and the goal of a diversity of freely given relationships.

Care Manager

The responsibilities of the Care Manager position include, but are not limited to direct case management, participation in care coordination via the treatment team and supervision of Shared Living and Adult Family Care (AFC) services. A clear and comprehensive understanding of DDS/ABI, AFC and other community/ home care delivery services preferred. The ability to adapt to changes, modify and redirect processes is required.  

This position combines a hybrid working model with ability to attend in person visits to homes and to meetings.

Requirements

  • Bachelor’s degree from an accredited college required
  • Two (2) years recent experience working with the elder population or those having a disability
  • A broad working knowledge of DDS or Adult Family Care community- based services is preferred
  • Computer skills, organizational planning, travel, as well as excellent communication skills and team building skills are necessary
